What is a granuloma?

A granuloma is an area of inflammation caused by immune system. It’s made up of a cluster of white blood cells that have been “walled off” from the rest of body to try to protect us from harmful bacteria and viruses. Granulomas are surrounded by fibrous (dense) tissue, which makes them feel hard and lumpy.

Löfgren syndrome?

Löfgren syndrome is a sudden onset of sarcoidosis defined by a group of symptoms affecting several parts of the body:

• Arthritis in multiple joints.
• Fever.
• Red, tender bumps on the shins (erythema nodosum).
• Swollen lymph nodes in the chest.

If not treated or treat allopathically, Löfgren syndrome usually resolves in six months to two years.

Who does sarcoidosis affect?

While sarcoidosis can affect anyone, it’s more common in Black people than in white people. It also affects those assigned female at birth (AFAB) slightly more than those assigned male at birth (AMAB). Most cases are diagnosed between the ages of 25 and 40.

The symptoms of sarcoidosis depend on where in our body granulomas form. Most people with sarcoidosis have lung symptoms, but one can have symptoms in his/her skin, eyes, joints and almost anywhere else in the body. Sometimes patient may also have general feelings of being unwell or no symptoms at all.

General symptoms of sarcoidosis

• Fever.
• Fatigue.
• Joint pain.
• Muscle aches or weakness.
• Night sweats.
• Swollen lymph nodes.
• Unexplained weight loss.
• Kidney stones.
• Cough.
• Shortness of breath (dyspnea).
• Chest pain.
• Wheezing.
• Blurred vision or loss of vision (optic neuritis).
• Eye pain.
• Red or swollen eyes (uveitis or conjunctivitis).
• Sensitivity to light.
• Growths under the skin around scars or tattoos.
• Light or dark patches of skin.
• Raised, reddish-purple sores or rash across nose or cheeks (lupus pernio).
• Red, tender bumps on shins (erythema nodosum).
• Chest pain.
• Fluttering heartbeat (palpitations).
• Irregular heartbeat (arrhythmia).
• Heart failure.
• Shortness of breath.
• Increased thirst or amounts of urine (Diabetes insipidus).

Stages of pulmonary sarcoidosis

Pulmonary sarcoidosis is sometimes described in stages called the Siltzbach classification system.

It’s important to know that these stages aren’t necessarily an indicator of severity, they’re based on patient’s lungs’ appearance on a chest X-ray and mostly describe where the granulomas are located.

Someone with sarcoidosis can move between stages or have granulomas disappear completely. The only stage that’s irreversible is stage four, which is permanent lung damage.

• Stage 0: X-rays don’t show any pulmonary sarcoidosis. Lungs and lymph nodes appear normal on an X-ray.
• Stage 1: Granulomas in our lymph nodes only.
• Stage 2: Granulomas in our lymph nodes and lungs.
• Stage 3: Granulomas in our lungs only.
• Stage 4: X-rays show pulmonary fibrosis, or permanent scarring of the lungs.

Sarcoidosis is usually diagnosed with a combination of a physical exam, imaging (such as a chest X-ray) and biopsy of suspected granulomas. Because sarcoidosis can look like other conditions, healthcare provider should perform additional tests to rule out other illnesses.

Tests required

Imaging tests and biopsies are the most common ways to diagnose sarcoidosis.

Biopsies can be done in many different ways, depending on where the doctor suspects patient have granulomas. Skin, conjunctiva (eye) and some lymph node biopsies can be non- or minimally invasive.

More involved procedures can examine tissue from the lungs using small tubes, cameras and biopsy equipment passed through the nose, or throat or an incision. These include:

• Endobronchial ultrasound-guided transbronchial fine needle aspiration (EBUS-TBNA).
• Bronchoscopic transbronchial biopsy.
• Mediastinoscopy.

Other tests for sarcoidosis include:

• Pulmonary function tests: Noninvasive tests that measure how well patient’s lungs work.
• Lab tests: Blood or urine tests are used to see how well patient’s liver, kidneys and other organs work.
• Electrocardiogram (EKG or ECG): A routine office test that checks the electrical activity of the heart.
• Nuclear imaging: PET scans and gallium scans usea small amount of radioactive material injected into a vein to show inflammation in the body.
• Purified protein derivative: A simple skin test that helps establish prior exposure or infection with tuberculosis (TB). This can diagnose or rule out TB, which is sometimes mistaken for sarcoidosis.
• Slit-lamp examination: An eye checkup to detect eye-related problems caused by sarcoidosis.

Allopathic treatment for sarcoidosis is usually to manage symptoms and prevent organ damage. Since granulomas are areas of inflammation caused by immune cells, sarcoidosis is usually treated with medications that modify or suppress immune system. Treatment depends on where granulomas are in the body.

There’s no specific cure for sarcoidosis in Allopathy, though it often goes away on its own.

Allopathic medications, used to treat sarcoidosis

Medications to treat sarcoidosis aim to reduce inflammation by modifying or suppressing patient’s immune system. Some commonly used allopathic drugs in sarcoidosis include:

• Corticosteroids: Corticosteroids, like prednisone and cortisone, are drugs that reduce inflammation in the body. These can either be pills or inhaler or topical creams.
• Immunosuppressants: Immunosuppressants help stop patient’s immune system from causing organ damage and inflammation. Methotrexate is an immunosuppressant commonly used in sarcoidosis.
• Anti-tumor necrosis factor-alpha antibodies (anti-TNF-alpha antibodies): Anti-TNF-alpha antibodies, like infliximab, target a specific chemical into the immune system to reduce inflammation (infusion).
• Nonsteroidal Anti-Inflammatory Drugs (NSAIDs): NSAIDS are medications that can give temporary relief of symptoms caused by inflammation.

Complications/side effects of allopathic treatment

Corticosteroids can put the patient at increased risk of getting sick with infectious diseases and can cause a number of side effects, for example:

• Excessive weight gain.
• Insomnia.
• Acne.
• Diabetes in susceptible people.
• High blood pressure.
• Glaucoma.
• Cataracts.
• Osteoporosis.
• Depression and emotional irritability.
• Skin bruising.

Allopathic treatment duration

Conium Maculatum

Cancerous diathesis. Arterio-sclerosis. Caries of sternum. Enlarged glands. swollen lymph nodes that are painful. In most cases needing it, the pain is tingling and shooting in nature. Graphites

Anemia with redness of face. Tendency to obesity. Swollen genitals. Enlargement of glands of axilla, groin and neck with skin symptoms. Sarcoidosis. Abdomen – large and hard. Cancer of pylorus. Duodenal ulcer. Constrictive pain in stomach. Recurrent gastralgia. Flatulence. Fullness and hardness in abdomen. Chronic diarrhea, stools brownish, liquid, undigested, offensive. Merc Sol

This medicine is mainly helpful for cases having swelling of inguinal lymph nodes (in the groin), Stabbing pain, with chilliness. The pain gets worse while walking and standing. Swelling of nodes in the neck. Liver enlarged; sore to touch, indurated. Jaundice. Bile secreted deficiently. Almost constantly moist. Vesicular and pustular eruptions. Ulcers, irregular in shape, edges undefined. Glands swell every time patient takes cold. Sarcoidosis. Buboes. Orchitis.

Stool: Greenish, bloody and slimy, worse at night, with pain and tenesmus.

Manses profuse, with abdominal pains. Leucorrhea excoriating, greenish and bloody. Mammae painful and full of milk at menses.

Fever: Generally gastric or bilious, with profuse nightly perspiration; debility, slow and lingering. Heat and shuddering alternately. Yellow perspiration. Profuse perspiration without relief. Creeping chilliness, worse in the evening and into night. Alternate flashes of heat in single parts.

Merc Iodide

Throat affections, with greatly swollen glands and characteristic coating of tongue. Worse, right side. Chancre; induration remains long time. Swollen inguinal glands, large and hard. Mammary tumors, with tendency to much warm perspiration and gastric disturbances. Lacunar tonsillitis. When only the superficial part of the tonsil is involved. Cheesy exudates with offensive breath. Swelling begins on right side. Small ulcers on posterior pharynx. Easily detached patches on inflamed pharynx and fauces; worse on right tonsil; much tenacious mucus. Sensation of a lump. Constant inclination to swallow. Sarcoidosis.

Belladonna

It has a marked action on the vascular system, skin and glands, glands swollen, tender, red. Glandular swelling threatening suppuration. They may be in the axillary region or swellings of the mammie which are heavy, red and sensitive. Another character is red streaks radiating from the inflamed gland with great heat and stitching pains. Parotid gland swollen. Sensitive to loud tones. Hearing very acute. Otitis media. Sarcoidosis.

Tonsils enlarged; throat feels constricted; difficult deglutition; worse, liquids. Sensation of a lump. Loss of appetite. Swelling of glands of neck. Pain in nape.

Abdomen: Distended, hot. Transverse colon protrudes like a pad. Tender, swollen. Stool thin, green, dysenteric; in lumps like chalk. Prostatic hypertrophy. Testicles hard, drawn up, inflamed. Nocturnal sweat of genitals. Flow of prostatic fluid.

Dryness and heat of vagina. Dragging around loins. Pain in sacrum. Manses increased; bright red, too early, too profuse. Hemorrhage hot. Cutting pain from hip to hip. Manses and lochia very offensive and hot. Breasts feel heavy; are hard and red. Tumors of breast, pain worse lying down. Badly smelling hemorrhages, hot gushes of blood.

Lapis Albus

Affections of glands, goiter, pre-ulcerative stage of carcinoma. Burning, stinging pain in breast, stomach, and uterus. Connective tissue about glands specially affected. Ailanthus Glandulosa

This remedy shows by its peculiar skin symptoms its pronounced power of disorganizing the blood, causing conditions we meet with in low fevers, low types of eruptive diseases, diphtheria, follicular tonsillitis, Streptococcus infection, Hemorrhagic diathesis, etc. Sarcoidosis. The skin appears livid or purplish; face dark as mahogany, hot; sordes; throat swollen, purple, livid; semi-conscious, delirious; weak pulse, general torpor and prostration.

Throat: Inflamed, edematous, dusky red. Much swelling, internal and external. Dry, rough, scraping, choking feeling. Neck tender and swollen. Hoarse, croupy voice. Tongue dry and brown. Teeth covered with plaque. Pain in swallowing extends to the ears.

Skin: Miliary, livid rash, returns annually. Large blisters filled with dark serum. Irregular, patchy, livid eruption, disappearing on pressure. Cold. Raynaud’s disease.

Iodum

Iodum is one of the top medicines for all glandular structures, respiratory organs, circulatory system the enlargement of glands, including tonsils, with withering of the entire body, the muscles shrink and all the skin wrinkles, except mammary glands which dwindle, all glands are enlarged. Glands indurated, swollen, painful, in neck, axillae, groin, mammie; pains lancinating, cutting, burning. Burning, rawness and fissures; moisture. Bubo. Night sweat fetid and profuse.

Burning in vagina and labia. Darting in breast; painful indurations in breast, especially right. Cancer of uterus, burning pain down thighs.

Rhus Toxicodendron

Throat: Sore, with swollen glands. Sticking pain on swallowing. Parotitis.  Pain in maxillary joint. Swollen face, erysipelas. Cheek bones sensitive to touch.  Lobules swollen. Discharge of bloody pus.

Swollen, red, edematous; orbital cellulitis. Pustular inflammations. Photophobia; profuse flow of yellow pus. Sarcoidosis

Nausea, vertigo, and bloated abdomen after eating. Swelling of inguinal glands. Pain in region of ascending colon. Diarrhea of blood, slime, and reddish mucus. Dysentery, with tearing pains down thighs.

Male: Swelling of glands and prepuce-dark-red erysipelatous; scrotum thick, swollen, edematous. Itching intense.

Female: Swelling, with intense itching of vulva. Pelvic articulations stiff when beginning to move. Manses early, profuse, and prolonged, acrid.

Extremities: Hot, painful swelling of joints. Pains tearing in tendons, ligaments, and fasciae.

Skin: Red, swollen; itching intense. Vesicles, herpes; urticaria; pemphigus; erysipelas; vesicular suppurative forms. Glands swollen. Cellulitis. Burning eczematous eruptions with tendency to scale formation.

Aconitum Lycotonum

Breasts inflammation. Swelling of the mammary glands. Constipation. Diarrhea. Glands, enlargement of. Hodgkin’s disease. Swelling of the cervical glands, axillary and mammary glands and even where Hodgkin’s disease has been suspected. Sarcoidosis. Purulent discharge from ears; redness behind them. Tension in facial muscles. Hard tuberculous pimples on face. Skin of face becomes light brown. Numbness of jaw. Aversion to food. Eruption of hot, red, slightly painful spots on legs and toes.

Itching at vulva. Menstrual blood fetid. Excoriation of bend of thigh after menses. Leucorrhea viscid.

Aethusa Cynapium

Burning and pustules in throat, with difficult swallowing. glandular swelling, where swelling of glands round neck like a string of beads. Photophobia; swelling of Meibomian glands. Violent vomiting of a white frothy matter.  Easy perspiration. Surface of body cold and covered with clammy sweat. Lymphatic glands swollen. Sarcoidosis. Itching eruption around joints. Skin of hands dry and shrunken. Ecchymosis.

Muriaticum Acidum

Uvula swollen. Ulcers and false membrane. Edematous, dark, raw. Attempted swallowing produces spasm and choking. Aphthous mouth. Gums and glands swollen. Fetid Breath. Sores on teeth. Papular and vesicular eruptions, with great itching. Sarcoidosis. Carbuncles; foul-smelling ulcers on lower extremities. Scarlet fever, livid, with petechiae; scanty eruption. Eczema on back of hands.

Manses appear too soon. Leucorrhea. During menses, soreness of anus. Ulcer in genitals.

Phytolacca Decandra

Syphilitic bone pains; chronic rheumatism. Sore throat, quinsy, and diphtheria. Tetanus and opisthotonos. Decrease of weight. Glandular swelling with heat and inflammation. It is useful for tumors of breast with enlarged axillary glands. Sore spot in right hypochondrium. Rheumatism of abdominal muscles.

Throat: Dark red or bluish red. Much pain at root of tongue; soft palate and tonsils swollen. Tonsils swollen, especially right; dark-red appearance. Shooting pain into ears on swallowing. Pseudomembranous exudation, grayish white; thick, tenacious yellowish mucus, difficult to dislodge. Tension and pressure in parotid gland. Mumps. Follicular pharyngitis.

Female: Mastitis; mammae hard and very sensitive. Tumors of the breasts with enlarged axillary glands. Cancer of breast. Breast is hard, painful and of purple hue. Mammary abscess. Irritable breasts, before and during menses. Galactorrhea. Sarcoidosis. Ovarian neuralgia.

Male: Painful induration of testicles. Shooting along perineum to penis.

Skin: Itches, becomes dry, shrunken, pale. Papular and pustular lesions. Most useful in early stages of cutaneous diseases. Disposition to boils, and when sloughing occurs. Squamous eruptions. Syphilitic eruptions. Swelling and induration of glands. Venereal buboes. Scarlatina-like rash. Warts and moles.

Apis melifistida

Lids swollen, red, edematous, everted, inflamed; burn and sting. Conjunctiva bright red, puffy. Lachrymation hot. Photophobia. Sudden piercing pains. Pain around orbits. Serous exudation, oedema, and sharp pains.  Membrane of mouth and throat glossy, as if varnished. Red, shining, and puffy, like erysipelas. Cancer of the tongue. Constricted, stinging pains. Uvula swollen, sac-like. Throat swollen, inside and out; tonsils swollen, puffy, fiery red. Ulcers on tonsils. Fiery red margin around leathery membrane. Sudden puffing up of whole body. Sarcoidosis

Edema of labia; relieved by cold water. Soreness and stinging pains; ovaritis; worse in right ovary. Dysmenorrhea, with severe ovarian pains. Metrorrhagia profuse, with heavy abdomen, faintness, stinging pain. Ovarian tumors, metritis with stinging pains. Great tenderness over abdomen and uterine region.

Extremities: Edematous. Synovitis. Felon in beginning. Knee swollen, shiny, sensitive, sore, with stinging pain. Feet swollen and stiff. Feel too large. Rheumatic pain in back and limbs; Tired, bruised feeling. Numbness of hands and tips of fingers. Hives with intolerable itching. Edematous swellings.

Gelsemium

Difficult swallowing, especially of warm food. Itching and tickling in soft palate and nasopharynx. Pain in sterno-cleido-mastoid, back of parotid. Tonsils swollen. Throat feels rough, burning. Post-diphtheritic paralysis. Tonsillitis; shooting pain into ear. Feeling of a lump in throat that cannot be swallowed. Aphonia. Swallowing causes pain in ear. Sarcoidosis. Enlargement and inflammation of lymphatic gland with fever without thirst coming at 10 and 11 am and completely subsiding by evening. Cerebral symptoms such as distressing in head, vertigo et., may be present.

Ammonium Carbonicum

Malignant scarlatina, with somnolence, swollen glands, dark red sore throat, faintly developed eruption. Uremia. Heaviness in all organs. Uncleanness in bodily habits. Swelling of parts, glands. Burning of eyes with aversion to light. Enlarged tonsils and glands of neck. Burning pain all down throat. Tendency to gangrenous ulceration of tonsils.

Male: Itching and pain of scrotum and spermatic cords.

Female: Itching, swelling and burning of pudendum. Leucorrhea burning, acrid, watery. Manses too frequent, profuse, early, copious, clotted, black; colicky pains, and hard, difficult stool, with fatigue, especially of thighs.

Stool: Hot, green, watery, fetid, slimy, with colic. Chopped white and yellow mucus like chopped eggs and spinach. Soreness of anus. Diarrhea during dentition. Hemorrhoids, with painful fissures.

Nipples inflamed; tender to touch. Infant’s breasts tender. Yellow, acrid leucorrhea.

Kalium Bichromicum

Cirrhosis of liver. General weakness. Mucous membranes everywhere are affected. Catarrh of pharynx, larynx, bronchi and nose, and a tough, stringy, viscid secretion is produced, which condition is a very strong guiding symptom for this medicine. Perforation of the septum. Chronic atonic catarrh. Polypus. Dilatation of stomach and heart. Supra-orbital neuralgia, right side. Eyelids burn, swollen, edematous. Discharge ropy and yellow. Ulcers on cornea; no pain or photophobia.

Parotid glands swollen. Uvula relaxed, edematous, bladder-like. Pseudo-membranous deposit on tonsils and soft palate. Sarcoidosis. Aphthae. Diphtheria, with profound prostration and soft pulse. Discharge from mouth and throat, tough and stringy.

Stitches in region of liver and spleen. Cutting pain in abdomen, soon after eating. Chronic intestinal ulceration. Soreness in right hypochondrium, fatty infiltration of liver and increase in soft fibrous tissue. Painful retraction, soreness and burning.

Itching and pain of penis, with pustules. Ulcers, with paroxysmal stitches; aggravated at night. Constriction at root of penis, at night on awakening. Syphilitic ulcers, with cheesy, tenacious exudation.

Yellow, tenacious leucorrhea. Pruritus of vulva, with great burning and excitement. Prolapses uteri.

Bromium

Swelling of ovaries. Manses too early; too profuse, with membranous shreds. Low spirited before menses. Tumor in breasts, with stitching pains; worse left. Stitch pains from breast to axillae. Sharp shooting pain in left breast. Glandular swelling. Sarcoidosis. Hardness of glands without tendency to suppurate. Useful for swelling of testicles.

There may be complications associated with enlarged lymph nodes. If the lymph node swelling is related to an infection that is not treated, then an abscess (a pus-containing cavity) may result, which may require incisional drainage and treatment. The skin underlying the enlarged lymph node may also become infected.

In other cases, the lymph node may become very large and compress other nearby structures in the body. This could be a serious and debilitating problem that may require immediate medical or surgical attention. For example, the lymph node in the underarm (axilla) can compress the blood vessels and nerves supplying the arm. An enlarged lymph node inside the abdomen may compress the intestines and cause an obstruction of the intestines.
