Most skin cancers are locally destructive cancerous (malignant) growth of the skin. They originate from the cells of the epidermis, the superficial layer of the skin. Unlike cutaneous malignant melanoma, the vast majority of these sorts of skin cancers rarely spread to other parts of the body (metastasize) and become life-threatening.

Other rare forms of skin cancer include lymphomas, Merkel cell cancers, and cancers of other tissue in the skin, including sarcomas as well as hair and sweat gland tumors.

Except in rare instances, most skin cancers arise from DNA mutations induced by ultraviolet light affecting cells of the epidermis. Many of these early cancers seem to be controlled by natural immune surveillance, which when compromised, may permit the development of masses of malignant cells that begin to grow into tumors.

There are several different types of skin cancers:

• Basal cell carcinoma is the most common cancer in humans. Over 1 million new cases of basal cell carcinoma are diagnosed in the U.S. each year. There are several different types of basal cell carcinoma, including the superficial type, the least worrisome variety; the nodular type, the most common; and the morpheaform, the most challenging to treat because the tumors often grow into the surrounding tissue (infiltrate) without a well-defined border.
• Squamous cell carcinoma accounts for about 20% of all skin cancers but is more common in immunosuppressed people. In most instances, its biologic behavior is much like basal cell carcinoma with a small but significant chance of distant spread.
• Less common skin cancers include melanoma, Merkel cell carcinoma, atypical fibroxanthoma, cutaneous lymphoma, and dermatofibrosarcoma.

The most common risk factors for skin cancer are as follows.

Most basal cell carcinomas have few if any symptoms. Squamous cell carcinomas may be painful. Both forms of skin cancer may appear as a sore that bleeds, oozes, crusts, or otherwise will not heal. They begin as a slowly growing bump on the skin that may bleed after minor trauma. Both kinds of skin cancers may have raised edges and central ulceration.

Signs and symptoms of basal cell carcinomas include:

• Appearance of a shiny pink, red, pearly, or translucent bump
• Pink skin growths or lesions with raised borders that are crusted in the center
• A raised reddish patch of skin that may crust or itch, but is usually not painful
• A white, yellow, or waxy area with a poorly defined border that may resemble a scar.

Signs and symptoms of squamous cell carcinomas include:

• Persistent, scaly red patches with irregular borders that may bleed easily.
• Open sore that does not go away for weeks.
• A raised growth with a rough surface that is indented in the middle.
• A wart-like growth.

Actinic keratoses (AK), also called solar keratoses, are scaly, crusty lesions caused by damage from artificial ultraviolet light or due to excess of sunlight, often in the facial area, scalp, and backs of the hands. These are considered precancers because if untreated, up to 10% of actinic keratoses may develop into squamous cell carcinomas.

Moles are almost always harmless and only very rarely turn into skin cancer. If a mole becomes cancerous, it would be melanoma.

There is a precancerous stage, called a dysplastic nevus, which is somewhat more irregular than a normal mole. An early sign of melanoma is noticing a difference in a mole:

• asymmetry,
• irregular border,
• color changes,
• increasing diameter, or
• other evolving changes may signify a mole is a melanoma.

Moles never become squamous cell carcinomas or basal cell carcinomas.

Skin cancers typically arise in areas may appear with only limited sun exposure such as the back, chest, or extremities. However, skin cancer may occur anywhere on the skin, because as I (Dr Qaisar Ahmed MD, DHMS) explained that “cancer itself is not a disease but a fungi (parasite). Parasites likes the hoter, humid and shady area.

A skin examination by a dermatologist is the way to get a definitive diagnosis of skin cancer. In many cases, the appearance alone is sufficient to make the diagnosis.

A skin biopsy is usually used to confirm a suspicion of skin cancer. This is performed by numbing the area under the tumor with a local anesthetic such as lidocaine. A small portion of the tumor is sliced away and sent for examination by a pathologist, who looks at the tissue under a microscope and renders a diagnosis based on the characteristics of the tumor.

There is no specific staging system for basal cell carcinoma.

• If the tumor is wider than 2 cm (about ¾ inch diameter), it is probably a more serious tumor.
• Basal cell carcinomas of the ears, nose, and eyelid may also be of more concern, regardless of the size.

There is a staging system for squamous cell carcinoma.

• Large tumors that are thicker than 2 mm, invade the nerve structures of the skin, occur on the ear, and have certain worrisome characteristics under the microscope are of more concern.
• If the tumor metastasizes to a site at some distance from the primary tumor, the cancer is likely to be a dangerous tumor.

There are several effective means of treating skin cancer. The choice of therapy depends on the location and size of the tumor, the microscopic characteristics of cancer, and the general health of the patient.

• Topical medications:

In the case of superficial basal cell carcinomas, some creams, gels, and solutions can be used, including imiquimod, which works by stimulating the body’s immune system causing it to produce interferon which attacks cancer, and fluorouracil (5-FU), a chemotherapy drug. Side effects of these topical treatments, may have redness, inflammation, and irritation. A drawback of topical medications is that there is no tissue available to examine to determine if a tumor is removed completely.

• Destruction by electrodesiccation and curettage (EDC):

The tumor area is numbed with a local anesthetic and is repeatedly scraped with a sharp instrument (curette), and the edge is then cauterized with an electric needle. The advantage of this method is that it is fast, easy, and relatively inexpensive. The disadvantages are that the scar is often somewhat unsightly, and the recurrence rate is as high as 15%.

• Surgical excision:

The area around the tumor is numbed with a local anesthetic. A football-shaped portion of tissue including the tumor is then removed and then the wound edges are closed with sutures. For very big tumors, skin grafts or flaps are needed to close the defect. The advantages of this form of treatment are that there is a greater than 90% cure rate, the surgical specimen can be examined to be sure that the whole tumor is successfully removed, and the scar produced is usually more cosmetically acceptable than that of the EDC procedure. It is a more complicated procedure and is more expensive than EDC.

• Mohs micrographic surgery:

The site is locally anesthetized and the surgeon removes the visible tumor with a small margin of normal tissue. The tissue is immediately evaluated under a microscope and areas that demonstrate residual microscopic tumor involvement are re-excised and the margins are re-examined. This cycle continues until no further tumor is seen. This more complicated and expensive option is the treatment of choice for tumors where normal tissue preservation is vital, where the tumor margins are poorly defined, in tumors that have been previously treated and have recurred, and in certain high-risk tumors.

• Radiation therapy:

Ten to fifteen treatment sessions deliver a high dose of radiation to the tumor and a small surrounding skin area. This form of treatment is useful for those who are not candidates for any surgical procedure. The advantage of radiation therapy is that there is no cutting involved. The disadvantages of this expensive alternative are that the treated area cannot be tested to be sure the whole tumor is gone and radiation scars look worse over time.

Radiation itself is the one of the main cause(s) of cancer and cam trigger the current cancer to dangerous stages.

Other types of treatments for skin cancers include cryosurgery where tissue is destroyed by freezing,  photodynamic therapy (PDT) in which medication and ultraviolet light (blue light) are used to destroy the cancerous tissue, laser surgery to vaporize (ablate) the skin’s top layer and destroy lesions, and oral medications vismodegib and sonidegib.

• The prognosis for nonmelanoma skin cancer is generally excellent.
• Both basal cell carcinoma and squamous cell carcinoma are highly curable.
• There are virtually no deaths from basal cell carcinoma and only rare deaths with squamous cell carcinoma skin cancers, mostly in immunosuppressed individuals.
• Depending on the method of treatment and the location and type of skin cancer, the likelihood of a recurrence of a previously treated skin cancer is as low as 1% to 2% for Mohs surgery and up to 10% to 15% for destruction by electrodesiccation and curettage.

Early detection of skin cancers can lead to better outcomes. Know your skin and if you have any moles or spots that are suspect, see a dermatologist for a skin cancer screening. Awareness is key in identifying and treating skin cancers early.

A major source of vitamin D comes from sunlight exposure, which leads to the production of the vitamin in the skin. Some argue sunscreens block out so much of the sun’s rays that inadequate vitamin D synthesis results. In fact, very few people actually apply sunscreen to every inch of their exposed skin, so vitamin D synthesis does occur. There is no reason not to use sunscreens because of a fear of low vitamin D. If there is a concern, vitamin D can be obtained by eating leafy vegetables or taking an oral multivitamin supplement.

Sodium bicarbonate is safe, extremely inexpensive and unstoppably effective when it comes to cancer tissues. Cancer cells cannot survive in the presence of high levels of oxygen. Sodium bicarbonate is, for all intent and purposes, an instant killer of tumors. The extracellular (interstitial) pH (pHe) of solid tumors is significantly more acidic compared to normal tissues.

PH controls the speed of our body’s biochemical reactions. It does this by controlling the speed of enzyme activity as well as the speed that electricity moves through our body. The higher (more alkaline) the pH of a substance or solution, the more electrical resistance that substance or solution holds. Therefore, electricity travels slower with higher pH.

If we say something has an acid pH, we are saying it is hot and fast. Alkaline pH on the other hand, biochemically speaking, is slow and cool.

When oxygen enters an acid solution it can combine with H+ ions to form water. Oxygen helps to neutralize the acid, while at the same time the acid prevents oxygen from reaching the tissues that need it. Acidic tissues are devoid of free oxygen. An alkaline solution is just the reverse. Two hydroxyl ions (OH-) can combine to produce one water molecule and one oxygen atom. In other words, an alkaline solution can provide oxygen to the tissues.

At a pH slightly above 7.4 cancer cells become dormant and at pH 8.5 cancer cells will die while healthy cells will live.

Sodium carbonate has been found effective in treating poisoning or overdose from many chemicals and allopathic/pharmaceutical drugs by negating the cardiotoxic and neurotoxic effects. Sodium bicarbonate is useful in treating neurological disorders in children.

Sodium and potassium bicarbonate helps to neutralize excess acids of any kind.

Vitamin D Deficiency Linked to Cancer

The main reason is that UVB rays from the sun produce vitamin D, and evidence links vitamin D deficiency with an increased risk of cancer.

In a 2008 case-control study published in the journal Carcinogenesis, researchers found that post-menopausal women with low vitamin D in the blood had an increased risk of breast cancer.

The study observed cancer patients between the ages of 50 and 74. In total, 1,394 cases and 1,365 controls were used in the study. Another case-control study from 2014 suggested that less than an hour of sunlight daily could reduce breast cancer risk.

Sunlight Kills cancer

As we know there is an anti-cancer link between vitamin D and breast cancer, but how?

The sunlight-induced vitamin D is thought to reduce the reproduction and spread of cancer cells, and increase the occurrence of mutated cell self-eradication.

Vitamin D has to be converted into 1, 25-dihydroxycholecalciferol in the liver and kidney in the presence of PTH. The 1,25-dihydroxycholecalciferol is the active product.

There are various forms of vitamin D. But, the most important one is vitamin D3 (also known as cholecalciferol).
Vitamin D3 is synthesized in the skin from 7-dehydrocholesterol, by the action of ultraviolet rays from the sunlight.

Activation of vitamin D3 occurs in two steps:

First step:-
Cholecalciferol (vitamin D3) is converted into 25- hydroxycholecalciferol in the liver. This process is limited. and is inhibited by 25-hydroxycholecalciferol itself by feedback mechanism.
This inhibition is essential for two reasons:
i. Regulation of the amount of active vitamin D .
ii. Storage of vitamin D for months together. If vitamin D3 is converted into 25-hydroxycholecalciferol, it remains in the body only for 2 to 5 days. But vitamin D3 is stored in liver for several months.
Second step:-
25-hydroxycholecalciferol is converted into 1,25- dihydroxycholecalciferol (calcitriol) in kidney. It is the active form of vitamin D3. This step needs the presence of PTH.

Role of Calcium Ion in Regulating 1, 25-Dihydroxycholecalciferol:-
When blood calcium level increases, it inhibits the formation of 1,25-dihydroxycholecalciferol.
The mechanism involved in the inhibition of the formation of 1,25-dihydroxycholecalciferol is as follows:
1. Increase in calcium ion concentration directly suppresses the conversion of 25-hydroxycholecalciferol into 1,25-dihydroxycholecalciferol. This effect is very mild
2. Increase in calcium ion concentration decreases the PTH secretion, which in turn suppresses the conversion of 25-hydroxycholecalciferol into 1,25-dihydroxycholecalciferol. This regulates the calcium ion concentration of plasma itself indirectly, i.e. when the PTH synthesis is inhibited, the conversion of 25-hydroxycholecalciferol into 1,25-hydroxycholecalciferol is also inhibited. Lack of 1,25-dihydroxycholecalciferol, decreases the absorption of calcium ions from the intestine, from the bones and from the renal tubules as well. This makes the calcium level in the plasma to fall back to normal.
Actions of 1, 25-Dihydroxycholecalciferol:-
1. It increases the absorption of calcium from the intestine, by increasing the formation of calcium binding proteins in the intestinal epithelial cells. These proteins act as carrier proteins for facilitated diffusion, by which the calcium ions are transported. The proteins remain in the cells for several weeks after 1,25-dihydroxycholecalciferol has been removed from the body, thus causing a prolonged effect on calcium absorption
2. It increases the synthesis of calcium-induced ATPase in the intestinal epithelium
3. It increases the synthesis of alkaline phosphatase in the intestinal epithelium
4. It increases the absorption of phosphate from intestine along with calcium.

A new study published in The Journal of Steroid Biochemistry and Molecular Biology last year found that vitamin D compounds suppress the breast cancer stem cell population, which may inhibit breast cancer growth.

Basically, radiation and chemotherapy do not shrink the tumor or kill the colony; the cancer stem cells remains but now more violent, which can cause an aggressive cancer recurrence in the near future. The chemotherapy and radiation pretty much just make the cancer stem cells angry.

The breast tissue abnormality ductal carcinoma in situ (DCIS) was once considered cancerous, but is now believed to be benign. DCIS may also progress to invasive ductal carcinoma (IDC), which is a more serious breast cancer risk, though it may not develop into breast cancer.

Conventional therapy “radiotherapy, chemotherapy, lumpectomies, and mastectomies for DCIS and IDC cases” do not works but aggravate the disease.

Other Ways to Reduce Cancer Risk

Vitamin D3 may be a safe and effective alternative or complementary treatment to conventional breast cancer therapy. It is also considered a cost-effective preventative therapy against the recurrence of breast cancer.

Decreased dietary intake and/or absorption, certain malabsorption syndromes such as celiac disease, short bowel syndrome, gastric bypass, inflammatory bowel disease, chronic pancreatic insufficiency, and cystic fibrosis may lead to vitamin D deficiency. Lower vitamin D intake orally is more prevalent in the elderly population.

About 50% to 90% of vitamin D is absorbed through the skin via sunlight while the rest comes from the diet.
